Enrollment Eligibility Requirements
Effective January 7, 2011
The Humanitarian Service Project has recently changed the program enrollment eligibility requirements for our Children’s Birthday Project.
The process of enrolling in our Children’s Birthday Project is as follows:
All families looking to enroll a child(ren) ages 3-11 must complete a referral form with an outside social service agency. These referrals can come from school social workers, police department social service departments, and townships.

In order to be eligible for the Children’s Birthday Project, the family must meet income requirements, and show proof of medical benefits through Illinois All Kids. Benefits must be at the “All Kids Assist” level. (This level of Kids Care requires no co pays)
Additionally, if you do not have an All Kids card, you may also become enrolled by providing proof from your child’s school that he or she receives Free Lunch.
You must send a copy of your All Kids card, or a copy of the letter from your child’s school proving that he or she receives free lunch with your referral form.
